Novo Nordisk announced topline results Monday from two phase 3 trials of CagriSema, the once-weekly combination of cagrilintide and semaglutide it has positioned as its next act in obesity and type 2 diabetes. The headline is a head-to-head win.

In REIMAGINE 5, CagriSema at 1.0 mg/1.0 mg produced an estimated average weight loss of 12.4% in adults with type 2 diabetes at week 60, against 9.1% for tirzepatide at 5 mg, and matched it on HbA1c reduction — 1.71% versus 1.67%. In REDEFINE 9, the same dose delivered 21.0% weight reduction at week 68 in adults with overweight or obesity, versus 2.0% for placebo, with gains across blood pressure, waist-to-height ratio, and fasting lipids. Both trials were presented at the company's Capital Markets Day in Bagsværd, Denmark.

The dose is the part investors will keep repeating. CagriSema beat tirzepatide using the lowest strength of the combination, which is the argument Novo Nordisk needs after a year of being read as the company that lost its lead to Eli Lilly. Tolerability was described as consistent with prior studies — no small thing in a category where side effects, not efficacy, drive people off treatment.

Commercially, the timing matters more than the percentage points. The obesity market has moved from scarcity to selection: supply constraints have eased, pharmacy benefit managers are negotiating hard, and direct-to-consumer pricing has turned a prescription drug into something closer to a consumer brand decision. A drug that can claim superiority over the incumbent at a low dose is a marketing asset as much as a clinical one.

The caveats are the usual ones. Topline results are not a filing, 5 mg is not tirzepatide's top dose, and cross-trial arithmetic has embarrassed better analysts than most. But after eighteen months of narrative losses, Novo Nordisk walked into its own investor day with a number it could say out loud.
